Face to Face - Return to Work

At Team Grant, we are dedicated to maintaining a positive and healthy workplace for our staff. In accordance with ACAS guidelines, we ask employees who have been absent to first self-certificate their absence using this form, followed by a face to face return to work interview (this will be scheduled at the earliest convenience of Team Leader (TL) and yourself). This two-step approach helps us:

• confirm that staff are ready and well enough to resume their responsibilities safely,

• allows us to plan any potential adjustments to allow your successful transition back to work,

• helps make the first day back smoother.

  If your absence is 7 days or less you are required to self certify your absence by completing this form. In the event that your absence is longer than 7 days a Statement for Fitness for Work form (“fit note") will be required. Information within this form will be handled confidentially in line with the GDPR.
